This form will allow you to submit an updated Serology Report to your Combat Sports profile.

Persons wanting to register or renew as a Contestant are required to submit medical information pursuant to section 16(2)(b) of the Combat Sports Act 1987.

The prescribed medical information required under regulation 8A(1)(b) is a serology report prepared within 6 months before the date of application, stating whether or not the applicant has; Human Immunodeficiency Virus (HIV), Hepatitis B, Hepatitis C.

Contestant will not be allowed to compete if their serology report is more than six (6) months old at the time they intend to compete.
